What is Transitional Planning?

Enhanced Transitional Planning is a ten month support program fully funded by the Ontario Trillium Foundation to develop person-centred transition plans for a limited number of people with disabilities and their families, built on a foundation of:

  • Vocation & community engagement
  • Relationships
  • Wellness & Recreation
  • Financial Security
  • Spirituality
  • Housing & Lifestyle


Goals & Outcomes

Brantwood Community Services believes that early intervention, enhanced discovery techniques and family/caregiver inclusive planning increases the independence and well-being of people supported as well as improving the skills of the person and their families. Our goal is to empower people of all abilities to have a life of their choosing.

 Participants & their families will;

  • Have access to information on adult services
  • Set goals for the family & individual
  • Create a crisis plan before you need it
  • Be connected with supports for the Individual & Family
  • Develop long & short term plans


What to Expect in Enhanced Transitional Planning

 An Enhanced Transitional Plan may include many of the following elements and more:

  • Person-Centred Discovery Process
  • Long Term Planning
  • Short Term Planning
  • Peer Group Coaching
  • Family Support Group & Coaching


How to Apply

Referrals to the program can be made by any individual with a disability or a family/caregiver. The program is for people with disabilities between 15-29 yrs old. Apply for your spot in our program by email to: Sherri Lojzer
